A. THỦ TỤC NHẬP KHẨU KHẨU TRANG N95 DƯỚI DẠNG KHẨU TRANG Y TẾ

HS CODE: 63079040. (Thuế NK: 20% ; VAT 5%). Lưu ý: nếu hàng có CO form E, form AK sẽ được giảm thuế NK.

Hồ sơ HQ gồm: Phân loại A, PTN Công bố A, INV, PKL, vận đơn, tem nhãn.

Hồ sơ Công bố tiêu chuẩn áp dụng TBYT loại A:

1. Văn bản công bố tiêu chuẩn áp dụng của trang thiết bị y tế thuộc loại A

2. Bản phân loại trang thiết bị y tế

3. Giấy chứng nhận đạt tiêu chuẩn quản lý chất lượng còn hiệu lực tại thời điểm nộp hồ sơ công bố đối với trang thiết bị y tế nhập khẩu.

4. Giấy ủy quyền của chủ sở hữu trang thiết bị y tế cho tổ chức đứng tên công bố tiêu chuẩn áp dụng

5. Giấy xác nhận đủ điều kiện bảo hành do chủ sở hữu trang thiết bị y tế cấp theo mẫu quy định tại Phụ lục VII ban hành kèm theo Nghị định 36/2016/NĐ-CP, trừ trường hợp trang thiết bị y tế sử dụng một lần theo quy định của chủ sở hữu trang thiết bị y tế.

6. Tài liệu mô tả tóm tắt kỹ thuật trang thiết bị y tế

7. Bản tiêu chuẩn mà chủ sở hữu trang thiết bị y tế công bố áp dụng hoặc Giấy chứng nhận hợp chuẩn.

8. Tài liệu hướng dẫn sử dụng của trang thiết bị y tế.

9. Mẫu nhãn sẽ sử dụng khi lưu hành tại Việt Nam của trang thiết bị y tế.

B. THỦ TỤC NHẬP KHẨU KHẨU TRANG N95 DƯỚI DẠNG KHẨU TRANG HÀNG THƯỜNG: Khẩu trang lọc bụi, dùng trong công nghiệp

Bộ hồ sơ làm đăng ký kiểm tra chất lượng:

1. 3 đơn yêu cầu kiểm tra chất lượng ( theo mẫu)

2. Invoice

3. Packing list

4. Vận đơn

5. Hợp đồng

6. Hình ảnh hàng

7. 2 giấy giới thiệu ( theo mẫu )

– Hàng về cần làm hợp quy bên QCVN:08. Để làm được hợp quy này thì cần có kết quả test Niosh trước ( Phí hợp quy sẽ báo theo lô hàng, và số model )

Hồ sơ đăng ký chứng nhận hợp quy :

Sau khi hàng về thì làm hợp quy

– Giấy đăng ký chứng nhận hợp quy (2 bản) Hợp đồng (Contract) (bản sao)

– Danh mục hàng hóa (Packing list) kèm theo hóa đơn (bản sao)

– Hóa đơn (Invoice) (bản sao)

– Vận đơn (Bill of Lading) (bản sao)

– Giấy chứng nhận xuất xứ (C/O-Certificate of Origin) nếu có (bản sao)

– Ảnh + bản mô tả hàng hóa sản phẩm (tiếng việt)

– Test reports của mặt hàng đăng ký (bản sao)

– Tờ khai hàng hóa nhập khẩu (có thể bổ sung sau)
